Job Description
There are 5 posts covering – Northwich, Ellesmere Port, Neston, Macclesfield and Chester, (employer is Disability Positive), to work as part of the Community Engagement Team, responsible for the effective planning and delivery of social activities for disabled people aged 5+ across the locality and wider community.
£19448 rising to £20420 FTE after successful probationary period
hours range from 1.5 to 21.5 per week (regular evening and weekend work)
To empower disabled people to access leisure and/or physical activities of their choice within their local community working across a number of Disability Positive commissioned and non-commissioned services.

•Support disabled children and young people to participate in activity sessions delivered by Disability Positive in the local and wider community
• Provide personal care, medication and support with behaviour for identified young people
• Support the Group Coordinator to consult with disabled children and young people, parents/carers and volunteers to inform the planning of all activity sessions
• Participate/deliver a user led steering group
• Ensure compliance with risk assessments and personal support plans for all activities, venues and individual disabled children and young people where appropriate
• Work in partnership with colleagues and community providers to deliver the Duke of Edinburgh Award scheme for disabled people
• Provide support for volunteers to carry out their designated role within activity sessions
• In partnership with colleagues achieve project milestones as set by the designated manager..
• Report any issues or concerns about individual disabled people including any health and safety or safeguarding concerns
